New Patient Registration

To register as a new patient you will need to live within our practice boundary.

Unfortunately in order to maintain levels of service to our listed patients, we have capped our list. This means that with the exception of First-Degree Relatives (Parent-Child etc.) living at the same address as a current patient, we will be temporarily disallowing all new patient registrations. This is to enable the maximising of care of our current patients at a time of a national GP shortage.

NHS England Practice Allocation

If NHS England have allocated you to Saxonbrook Medical, please complete the below registration form.

We ask you to upload the following documents with your registration form:

  • Your Allocation Letter
  • Your NHS number, found on most medical letters or from your current GP
  • A recent proof of address, such as a council tax statement or utility bill
  • A form of photographic ID

Although it will make your registration process smoother, we will not stop you from registering with us if you do not have any of these.

If you prefer you can attend the surgery with your registration form and supporting documents and hand into a member of the team at reception.
