New Patient Registration
Unfortunately in order to maintain levels of service to our listed patients, we have capped our list. This means that with the exception of First-Degree Relatives (Parent-Child etc.) living at the same address as a current patient, we will be temporarily disallowing all new patient registrations. This is to enable the maximising of care of our current patients at a time of a national GP shortage.
If you are not currently registered with a GP surgery, please visit www.nhs.uk/service-search for information on how to find an alternative provider.
NHS England Practice Allocation
If NHS England have allocated you to Saxonbrook Medical, please download a registration form and return to email@example.com.
It will assist us if in your reply you can provide:
- Your NHS number, found on most medical letters or from your current GP
- A recent proof of address, such as a council tax statement or utility bill
- A form of photographic ID
Although it will make your registration process smoother, we will not stop you from registering with us if you do not have any of these.
Before we can accept your registration we may also request to verify your identity, either in person at the surgery or via a video chat.
