Informed Consent for withdrawal from the Cervical Screening Programme

If you would like to defer/withdraw from the cervical screening programme please fill out one of the appropriate forms below and register your withdrawal/deferral on the CSAS link. Screening – Cease/Opt Out · CSAS.

In addition, please inform your surgery of your decision after submitting your form to mitigate inviting you inappropriately.

If you are still unsure on your decision, please feel free to discuss further with a member of our nursing team at the surgery or see the helping you decide leaflet (www.gov.uk – Cervical screening: leaflet for women considering screening) to support you in making your decision.

Please note that at any point you wish to opt back into the programme you can do so by completing a ‘Reinstate into Cervical Screening Programme’ form (www.csas.nhs.uk.docx (live.com)) and informing your surgery.
