Social Prescribing
Social Prescribing is a way of helping patients improve their health and well being by connecting them with activities/services happening in the local community. A simple example would be signposting somebody who has been diagnosed with dementia to local dementia support groups.
Social prescribing works for a wide range of people, including those:
- With one or more long-term conditions
- Who are lonely or isolated
- Living with low level mental health issues
- Have complex social needs which affect their wellbeing eg debt/benefit issues
- Who are Caring for others
- Who want to make lifestyle changes such as losing weight, giving up smoking and doing more exercise
You can ask to be referred to our team of Social Prescribing Link Workers. They will give you time to talk about what matters to you and connect you to community groups/activities/services that can help by offering practical or emotional support. After an initial conversation, Link Workers will suggest possible ways of support and encourage people to explore them for themselves.
Speak to your GP, Nurse or Reception Team to find out and to ask for a referral.
